First visit checklist

What to bring on your first session.

We can guide you on each item by phone or at intake. Missing something? Bring what you have - we'll work through the rest at the center.

  1. PhilHealth Member Data Record

    Bring a printed MDR. We can help reprint at the center if your last copy is outdated.

  2. PhilHealth Dialysis Database registration

    If you're already in the system from a previous center, bring proof of registration.

  3. HD Prescription from your nephrologist

    Bring your most recent prescription. Our medical team coordinates with your physician for handover.

  4. Recent labs + Hepatitis Profile

    Bring labs from the last 90 days plus your Hepatitis B and C status.

  5. Treatment Sheet

    If you've dialyzed elsewhere before, bring your existing treatment sheet for continuity of care. New patients receive one at the center on their first visit.

  6. One valid government ID

    PhilSys, Passport, Driver's License, or any government-issued ID.
